Boulder County & North Metro Neurosurgery
Neurosurgical Referral Information
A collaborative resource for clinicians referring patients for neurosurgical evaluation and care in Boulder County and northern Colorado.
Featured Clinicians
Our clinical team includes Chih-Ta Lin, MD, FAANS and Peter Lennarson, MD, FAANS, both board-certified neurosurgeons; Nancy L. Lennarson, MS, RN, AGACNP-BC, a board-certified adult-gerontology acute care nurse practitioner; and Terri Baker, PA-C, a certified physician assistant.
Comprehensive Neurosurgical Evaluation and Specialty Coordination
We welcome referrals for the full spectrum of neurosurgical conditions, including conditions that may fall outside the primary clinical focus of our local team. Referring clinicians are encouraged to contact us regarding complex cranial and spinal pathology, including cerebrovascular conditions, functional neurosurgery, and other specialized neurosurgical disorders.
Our goal is to provide patients and referring physicians with a knowledgeable, accessible point of contact for neurosurgical care. When a patient's condition requires expertise or procedures not routinely available through our local clinical services, our team can help facilitate evaluation and referral to appropriate neurosurgical subspecialists at the University of Colorado Anschutz Medical Campus.
This collaborative approach helps patients access the appropriate level of specialized care while maintaining communication and coordination with their referring clinicians.

Helpful Records & Imaging
Relevant referral notes, the clinical question, pertinent examination findings, available imaging and reports, and a summary of prior treatments can help the receiving team assess the referral. Include prior operative information when relevant.
Urgent & Routine Referrals
Routine outpatient referrals are not an emergency response service. For medical emergencies, activate emergency services or direct the patient to emergency care. For urgent but non-emergency clinical concerns, contact the receiving clinic and clarify the appropriate pathway.
Please contact UCHealth DocLine at 844-285-4555 for direct consultations with our neurosurgeons, clinician transfers and direct admissions to UCHealth hospitals, including Longs Peak Hospital. DocLine is an institutional consultation/transfer/admission service.
Communication & Follow-Up
Clear referral questions, an up-to-date referring clinician contact, and relevant records support collaboration. Ask the receiving clinic how consultation findings and follow-up plans will be communicated. Practice-specific turnaround times and notification workflows are not confirmed and are not promised here.
